I use my mouse (no joking)
I've changed my settings in GIMP to add weight and delay to the cursor, which allows me to make smooth lines.
Besides that, I also use a lot of polynomial curves (splines) for the extra curvy regions (because I'm an engineer who's used to 3D design software, I'm more comfortable using splines than a pen.)
