Just another bleak moment in the grim darkness of the far future. I wanted to show that Blotaria, while certainly a jolly lass happy to spread her god's gifts, is actually a terrifying presence to encounter for an average Imperial citizen and that meeting her would probably bring them nothing good.
But one can always reject the light of the Emperor, drop on one's knees and find an appreciation for Nurgle's blessings - at least becoming a willing Chaos cultist might be better than being taken as a slave on a traitor Astartes ship, or worse, a test subject for new plagues. Might be better...
