i don't know enough about horses to make an educated reply regarding the health of the horse
but, from my experience with dogs, particularly game bred APBT's, who most people think are 'too skinny', the animal is perfectly conditioned, people don't know about the breed, the dog was breathing in when the photo was taken, etc etc.

You should not see a horses ribs, neck or head like that.
If I ever saw a horse looking like that I would be calling law enforcement so they could investigate the horse and the conditions its living in.
The horse could be extremely old and its really hard to keep weight in it . But its best to to be safe than sorry.
