There is a Puerto Rican member club in the FIA (https://www.fia.com/about-fia/member-clubs/federacion-de-automovilismo-de-puerto-rico), so it should likely be possible to drive under the Puerto Rican flag, similar to how there is a Puerto Rican national soccer team because their federation gets recognized by FIFA.
But a Puerto Rican driver could probably also choose to drive for the US, if ACCUS is fine with it. Seeing Daniil Kvyat currently drives with an Italian license without being an Italian citizen, I guess a Puerto Rican driver would be able to have their choice here.

Like Gaskari said, it really depends on FIA licencing/federation recognition etc
One interesting example that comes to mind is Sebastian Montoya in F2. He races under a Colombian licence but on his car the flag is divided diagonally and the other half is the American flag.
