Balto prepares for some cargo delivery by pinching his nipples to stiffen them, allowing the nipple rings to be inserted, as he has already pierced his stiff nipples before.

I really like the pose and the expression on his face, as well as the anatomy.

But I'm a little confused... is something coming out of his nipples? If so, what is it?

Nope, that's a white ring attatched to the nipple ring. He's just pinching his pierced nipple so that he can attatch the piercing. The white ring is insterted in his finger.
