I get irritated every time I fill out a web form that gives me limited
choices to describe myself. Nowhere do I get the chance to assert to the
American Association of Petroleum Geologists that I AM A NATURALIZED CITIZEN OF SATURN or anything like that. The good news is that nobody believes the contents of databases, anyway, except for other databases. So there's already 42 different versions of you floating around the Internet.

If I was designing a user profile database, I'd let you choose to call yourself:

Mr.
Ms.
Miss
Buster
Mistress
Conquistador
Memsahib
(etc.)
