
Something I've noticed as an issue in my own art and others - though now I try to keep aware of it, which is progress.

I spent a season working on the Dilbert animated cartoon and always had problems with this. Those character's hands are freakin' TINY.
Consistantly I'd quickly rough out my scene, and then immediately go back thru and redraw all the hands half the size I'd originally drawn them so they'd be on model. :P

Foreshortening is incredibly hard to show. Even the most proficient artists struggle with it. Most people avoid it, or find some personal workaround. It can look awkward because you are drawing attention to the fact that the image is a 2-dimensional thing trying to show a 3-dimensional phenomenon. Even when it's photographically perfect, foreshortening is weird for the eyes/brain to understand.
I would simply avoid it. There are so many other poses and POV that can show whatever it might be, or communicate the same point without the awkwardness.

Push it so that the hand/foot pretty much entirely obscures the leg/arm, then you can fake it a lot more easily. Especially if you exaggerate the perspective - my rule of thumb for foreshortening is "draw the near part twice as big as you think you have to - even when taking this rule into account".
