I'd say it's risky since you need a unicorn who's very experienced both with healing magic AND as an ordinary doctor, so you don't risk, say, bones healing crooked in a few seconds as opposed to healing crooked over a matter of days. So they generally keep such doctors trained for the heavier stuff like cancer that can't be cured by mundane means.
Sure, you COULD get a unicorn friend to cast a growth spell on you to grow back those feathers, and if they roll a 1, oh well, you've got feathers coming out of your nose for a little bit... but a doctor/whitemage wouldn't risk a malpractice lawsuit for a miscast spell, over something that's going to grow back in a month anyway.

I also think that part of it has to do with the fact that all unicorns are a little bit crazy. Not necessarily a bad thing, as they are obviously still able to function within society, but it would tend to make things like magical recovery from any malady a bit rare, whether because the unicorn in question didn't think to try it, or because their patient didn't quite trust them to carry it out.
