The smaller guy is adjusting the collar (that's connected to the big guy's body and collects data and controls his temperament). The big guy is kind of a pet/tool/study subject. There's some other info in here http://hurtanminttu.tumblr.com/tagged/gregor but I have no proper extensive background written out yet.
Well it's more to do with the fact that he's there while someone's poking his spine, since he isn't in that position or wearing a collar because he wants to, (though there's bit of a compliment there too since he actually acknowledges wanting to kill the doctor, meaning that for someone so small and weak he's a threat enough to be killed).
