Can't answer the first three, since that's the ideas of the artist and his preferences, but the fourth one can be answered with "everyone is different". People do not all like the same thing - otherwise culture would be rather boring. This is what the artist likes, and what he chose to do.
It would be equally possible for me to ask why you like the concept of "fatties". But if I did, I wouldn't phrase it like that - the incredulity makes it seem like one thinks it's a "wrong" thing to like.

Physical or severe mental harm to a real person who is not a consenting adult. So BDSM is fine, say, but the existence of the "safe word" is to make sure they are still giving consent - once the safe word is given they are no longer consenting and continuing is wrong.
(And simply looking at pictures and being shocked does not count as mental harm, at least not unless the picture is wrongly tagged at a minimum. An argument could be made that this picture should be tagged "mature", but that depends on the current rules for mature/adult uploads.)
