This was a neat little giftart for the collection that
did. (I don't think he's actually open for commissions though)
The Helm of Opposite Alignment is a neat cursed item with debatable usefulness - on the one hand, it can make an evil, vicious gnoll like this one good-hearted and pious.
But it can also make a paladin turn villainous! And taking it off doesn't help, it's actually a one-time use item and stops being magical once it has done its thing. Which is permanent.

Then it pushes them into an alignment extreme. Personally I'd say that whatever their natural tendencies are strongest in, it reverses them and enhances the result.
Example: Lawful neutral druid, generally sticks to his grove and discourages visitor, though generally without violence unless necessary.
Enter helm, he becomes Chaotic Evil, and leaves his grove actively persecute the decadent city folk. He would probably lose some class abilities for not having a neutral alignment but he'd figure something out.
