Which order form type to do like better?

Please let me know what you think in the comments below! I know which I'm leaning.

Yes, I'm taking it old school (for my business history anyway).
From 2010 to now, my evolution has been: email, forms, blog store, google store, google forms, site store & site forms, ...+journal, and now to Store + (off site) forms
I'll NEVER go back to email, nor "journal". I find email to be incredibly inefficient, more work on my end, and just unprofessional to be honest.
The journals was a close second to that, but i don't recall ever really trying it, so I gave it a shot. And no like. Actually confirmed something I find disheartening.
I prefer the store, and the forms on my site. Everything in one place, organized, efficient, and other perks for both me and the client.
But alas...I feel it best to revise the forms: consolidate (cause it was just convoluted, i know), and move off site (now that off site forms accept payments.).
SO my compromise is revisiting forms!

JOT ver I think is good for not being too overwhelming to newcomers, but overall jot ver 2 isn't that much more overwhelming and it's nice to have most things on one page where you can more easily review and change them.
