Or is that vulneraBALLity?
This is relatively mild, or might seem so. It would most likely be a series of frames, like a comic page.
The first panel shows a fellow being fitted with a rather thick elastic band - but not one tight enough to geld. Just something so when hit, his balls cannot move out of the way of the blow. It also will keep his penis clear so that blows to the balls are all to the balls.
Further panels would show him going about the day (nude, in briefs, or even fully clothed depending on the world setting) and getting accidental blows (and maybe some on purpose...). Things such as doorknobs and drawers are at just the right/wrong level and so on.
Alternate version: Same first panel, and there is indication of Ring of Vulnerability (if briefs... fully clothed seems unlikely for this version) and further panels show gal(s) gleefully taking advantage of his advertised heightened sensitivity. The last panel might be a knockout. Or a hint that the next page would reveal it was rather... final.
