If I can remember from a classic cartoon, the cloth would be folded into a triangle, with two corners that function as the side, and the top corner as the front, and I think they put the side corners on top of the top corner, before holding them together with a pin.
There's the classic cartoon look as mentioned above so they end up with one big pin holding both sides in the middle.
There's also the option of giving them a fold that creates a "wing" on each side that wraps around and sits just overtop of the middle part on the front, similar to the sides of a disposable. In this case, the sides are then held to the front by a a pin on each side, so you end up with two pins, and a shape similar to disposables.
Those are certainly options - I believe there are ones with buttons in that shape. I know there are ones with snaps, and there are definitely ones with velcro in that style.
I don't.. Have you seen HTF Cub's diaper? It's cloth diaper and it has pin that stitched on them. Maybe you should tired to doing that if you considering drawing adult characters in them.
