I've revised the commission form since some of its information was no longer relevant and it was starting to cause a bit of confusion. (If you've already submitted a form, you do not need to resubmit it or make any changes.)

In addition to the revised form, I'm offering an alternative method to commission me if you don't want to use a Google Account. Just private message me and we'll work out the specifics there. It will take a bit longer since you'll be answering the same questions, so if you don't have any issues with the form, please use that instead.
