only advice i can give is just to be there for her and offer support... sometimes all a person whats is for someone to listen... and through listening you can usually figure out a way to actively do something to help

I just read a memoir called: SHE'S NOT THERE: A LIFE IN TWO GENDERS, by Jennifer Finney Boylan.
In the book, the author recounts her experiences growing up as a man, and her eventual transition into a woman. I thought it was a good read, and it was very insightful. It's available on Amazon.com for cheap, especially if you buy used:

No problem. I think it's worth checking out. I can't promise reading the book will fix anything or make things any easier, but sometimes it's comforting to know there are those out there who share similar experiences, so you are reminded you are never alone. The book deals a lot with how the author handled relationships with friends and family, so I would imagine there is at least something worth investigating here.

its all the places around her. she has literally applied in every place she can.
