Appropriate wait time

Sometimes you just need a microscope
This is just more of a broad spectrum question in regards to appropriate wait times on commission work. While I do most certainly understand that artists lives can be chaotic and that off and on they take on significantly more then they can chew, I do find myself wondering exactly how long is too long to wait?
So far the longest wait times went to a pair of quite delightful pics done by Kevin which are now in my gallery, http://www.furaffinity.net/view/6790898/ and http://www.furaffinity.net/view/6743868/ at almost a year to the date of requesting it. ((Thankfully she did keep frequent contact with all of her commissioners, she had swamped herself, I am eternally grateful for the updates))
The second piece which is now going on nine months I've not so much as gotten any progression notes from the artist, no indication of where in the line I sit nor even so much as a rough wire-frame outline. I have sent in a 'check up' not every roughly every two months to see where things are. I understand they have moved recently but as stated, any sign of progression has yet to be stated, shown, explained ect.
At what point is it fair then to...
A.) Request the piece finished or at least a bloody progress piece
B.) Request a refund
C.) Turn them over to the FA mod staff for noncompliance
I would Really quite like to avoid B and C as I am very fond of the artists work.

I've gone a year for one piece myself. By the time I got it I was like, "yeah thanks, whatever"
I'd say it depends on the level of communication.
After a month, if you haven't heard anything or gotten a sketch it's fair to ask for an update.
If they say it hasn't started you'd be within your rights to ask when they think it might be done.
Depending on their answer (or if they fail to deliver on their answer) then I'd say you're okay asking about a refund.
But, like I said, it depends on how you relationship with that artist is. Waiting 3 months might not be a big deal, but if you're waiting 6 months for a sketch I can see getting a little bent out of shape.

About the refund, if I asked, I sincerely doubt I'd get it, again moving tends to make you poor for a while. Truth be told I'd much rather have the piece then the money back.
The two pieces I'd waited a year for, as stated earlier, the artist kept in good contact with her commissioners and there was progress through it all, which is why I did not at all mind, she'd somewhat unintentionally swamped herself.
