Been in a box by myself for six hours and had time to think.
With the different transformations, anthro, ar, ap, tg, feral, taur, growth, shrinking, and the like one thing I haven't ran across is what happens with body modifications.
I mean like earrings and most piercings would stay the same. But what about like nipple piercings on a feral morph that doesn't have nipples on the chest(like horse or cows). Would the nipples migrate to where the udders are or just fade into the chest leaving the piercing in the skin? Also what about gauging in humans that is a fatty bit of skin but most animals have just cartilage. So what happens then? Holes in the ear cartilage or in the loose skin near the ears?
Next what about teeth and bones. Most people have at least one cavity filled, so what happens with a were/anthro/feral transformation. Some of the main stream transformations have the teeth fall out being replaced by the more animalistic teeth, but what of the ones where the teeth reform? Would the filling break the tooth, fall out, or just be painful as all hell? Then what if you had a broken bone where it needed a steel plate or screws. What would happen with the shift in bone length? Just have it rip the bone/limb apart in the growth/shrinking?
And finally plastic surgery. How would a nose job carry over to a anthro/feral, would the cut off bits regrow or would the snout/muzzle be really deformed from the bits missing? And finally boob jobs. Now the anthro transformations wouldn't be affected too much, but an AR, tg, or feral how would those sacks of saline react? Well a tg transformation would just carry over to moobs, but how would say a wolf transformation deal with that? Different chest shape and all? Or with the AR how would a child shape hold up to that?

I'm personally a fan of analogous parts shifting, so to answer some of your questions:
Breasts would move to their new position, so piercings would move with them. So the top-most teats on a feral or cow or something would have the piercings. Ears would have the holes, probably in loose skin.
The material filling a cavity would still be present in new teeth, just the exterior has changed, so new enamel forms to give it the proper shape. (Or could do the "fall out and replace" method, in a more scientific setting, since there's no real way to add enamel and such.)
Bones with braces/pins would grow in other areas, since the material prevents that area from changing shape, or would heal and render the material unnecessary.
Really, though, I'd ask myself how it works in the setting (Is it a form of healing magic, is it a body replacement, is it a serum, is it nanobots, etc?), and then use those answers to answer the other questions.
But maybe I'm overthinking it. ^_^;;
