Neither! It's a dermal anchor, which is designed to sit under the skin and look like you have a disk/spike/jewel/etc. screwed into your body :D

It would be like having a surface piercing, except there's no bar that goes all the way through the skin, and there's only one entry/exit hole. So the rejection rate is ridiculously low as well.

Weird thing: a really old character I had for a Magic Knight Rayearth RP I was in had three of these. Except it was a guy, and they were huge opaque discs as big as his fist.
