Alrighty~ My awesome artsy friend 
and a few others like 
recommended I get myself a commission form! So I did! Something simple for you to fill out if you'd like to get a commission from me!
But I want to make sure it is completely automated so I decided on doing a promotional thang! Here's how it works:
I have 10 slots for (only) sketch commissions open and no more. Of those 10, 2 of them will be raffled off for free through a random number generator. Once it is filled up I will close up the commission slots and I will draw them in stream which will be announce in my submissions when I start!

Now I also have this other thing open! It's available for anyone who has ever commissioned me and will be continuously available!. I'm VERY interested in your feedback so I know just what I'm doing right and what I'm doing wrong. So I have a survey that I'd love for you to participate in! 5 quick questions about my general performance and they're completely anonymous! I want you to be as candid as possible! Don't like something! Let me know!

I just want to said that always a pleasure to work with you <3
Maybe you should do some modifications :
- Make all the boxes you really need mandatory (description)
- Maybe add a small box to link a ref like (Do you have a ref and 2 boxes "No" and "Other" to put the link in there)
I would have linked you Cubi's form which is pretty complete but she removes it when she's no longer taking commissions...
And that's okay! I'll make do!
