I need some help regarding my dog's chewing issue.

During the summer, my dog always gets stuck with the fleas but that’s just a normal occurance every year. Now it’s winter and freezing cold outside, yet ever since the fleas left , he continued to chew on himself non stop to the point where he’s yelping out in pain because his skin is getting dried out and scratchy from it scabbing up. Originally the vet has given me the medication needed for him to fight off an infection and the fleas he had obtained from constantly chewing himself but now all the medication has been taken and he till continues to chew.
I then was told that he may have a skin allergy related to bug bites or him chewing in general so i bought some of the oatmeal shampoo and I bathe him at regular intervals with it ( about every weeks or so) and it seems like only for the rest of that day, he will stop chewing himself but the every next morning he gets right back to it. My housemates and I figured that sticking a cone on hus head would help force him to revocer because he can’t reach his tail but then the dog woulc go out of his way to scratch his lower back and tail against the wall or on the edges of furniture to make sure he can scratch himself, doing further damage in the process.
He ended up warping the cone to the point where all he has to do is angle his neck to the side, therefore he is able to continue chewing himself, even with the cone on. I know this is different from my usual posts but I’m not too sure what to do about this issue. I’m planning on taking him to the vet once again but I’m afraid all they will do is just give him even more medication that isn’t going to do it’s job like the first set I want to do what i can that hopefully doens't involve drugging him up to death. Does anyone else have any possible ideas with this?
