Like one to fill out to order from me or is that something you guys actually hate doing because it feels less personal?

I think you should do it. Because it does help you to know what the commissioner needs. Unless you're offering multiple slots or low priced commissions with your artistic freedom, then there's no need for it.

I can only recommend it. :) For me it's always annoying to realize in the middle of working that there are infos missing or having to go through notes and emails trying to catch all the important stuff that was said. I mean, that can still happen, but at least the start is much smoother that way.
