How should i write my resignation letter?

right now im feeling depressed and stress at work due to working overtime, lack of staff, and lack of break. and also im having family problem
and right now I need to write a resignation letter which contain the following
1. Working Overtime, and working too much days when I request 3 days only
2. Short staff in shifts due to people calling sick/not bother showing up
3. lack of break due to short staff and poorly rota done
And some point I have like family problem and right now I cant deal with much stress. I will add my family problem in my letter after these things listed sorted.
