Duncan sent me the scan a few minutes ago in IM. I took that color scan into Photoshop, adjusted the brightness/contrast, used the dodge/burn tool to help remove the dark shadow that shows up near the spine of the book, then slap it down to greyscale for the final product.
All my sketches are actually done in blue pencil and then shifted to greyscale for online use.

Blue pencil (mechanical and normal) are typically wax based instead of graphite. That means they don't smudge with your hand by rubbing. You can still erase them if you are light enough, but really, mostly you would just ink right on top of them and scan only the inked part.
