The text is a poem by Kalonymus ben Kalonymus in the 1300s about being trans... "Because we bless our misfortunes as well as our fortunes, I offer, 'Blessed be you, o G-d, who has not made me a woman.'" This stuff has always been around.

I don't speak Hebrew, so I had to take his word on what all the puns meant.
There's so many puns in this. The shoulders, Chokmah and Jehovah, are "clowns" to the left and "jokers" to the right, just like the text, and the bottom, castrated part of the tree, "Malkuth," Israel, is both a play on circumcision, and the part of the tree identified with the feet, which is... also a Hebrew genital euphemism (sensitive and they smell bad). It also has things to do with manifestations and dynasties and stuff. I'm no Kabbalist, I couldn't say for certain.
The little arrow is "mishoqvolet," roughly, a sinker or a plumb bob, but also a "dragging down" part.
Oh, and the word in the middle is a bizarre portmanteau roughly translating to "youterus."And where the ovaries would go, it's something like "bloodsquirters." Jesus, who knows. Whoever here can read hebrew will laugh more than I will.
