Commission Sheet
I have a nifty character data sheet I made up for cons some years ago that I have people fill out for commissions. It helps the customer get down in writing what they have in their mind regarding leg type, hair length, breast/penis size, etc. But what I didn't have was a good way of tracking such info on my side for doing commissiosns through FA or other source. I wanted a single tracking form for an artist to be able to fill out that would help keep a commission status up to date and hopefully have all the information needed in one place.
This doesn't replace the Character Data Sheet- this is for th artist to use. I'm going to try this with a dedicated notebook that will let me use some clear vinyl sheet protectors that will hold this form, printouts of the character's other artwork / references, etc. When it's done, I have a packet I can send to the customer in a handy clear protector.
I'm still tinkering with this form. If you want to use it, feel free- I've made the printable full resolution version available online at:

Enjoy!

The history of the government facility here that houses it is really colorful because it was first built during the Civil War and designed by Fredrick Law Olmstead (who's also famous locally for parks in Louisville, Ky. and more famous for New York's Central Park). That initial building now houses City Hall and a shopping district. The warehouses build later (and which during World War I was the largest shirt factory in the world and in WWII was a POW camp) are now the Census facility, Senatorial offices and an industrial park.
