It is indeed hard on the back, that's what the adjustable brace is for, lowering the upper body to help with spine.

Ooh I love that wheelchair design. Usually when I see characters like that with similar bodies using a wheelchair it makes them sit normally and it looks uncomfortable.

I do agree, the normal wheelchairs are too small and the mermaid body is not meant to oriented in L shaped at pelvis. The body rests best at obtuse angles, which the brace provides a raising position for the upper body. The wheel chair also allows for the mermaid to change laying position to rest on the side or back as well. The brace can also lower for the mermaid to be at flat position while keeping the body at safety.
