I'm actually still a virgin. But, when the time comes, I honestly don't see why you would use a condom for oral if you're supposed to be close to the person and wouldn't mind. Do you find condom use detracts from the story?
I've never read a fanfiction, story, or comic that had condoms in it, actually. But, speaking on assumptions, it really depends on how it was written. If it's a short part that explains putting it on and then rolling onto whatever task they characters were getting into, I don't think so, but if it's like 10 pages of condom explanation and 3 other pages of anything else, yeah that's definitely just a train wreck into the condom isle of a sex shop.

IS there just no disease in the fantasy worlds portrayed?
Again, it depends on how it's written. I don't think anybody ever pays attention to diseases and acts like they don't exist in pornography, besides the actual storyline hurt/comfort ones, because diseases aren't "sexy".

Again, of course it depends, but in general, I don't think so. In fact if anything it would make it better due to adding just that little bit of personality, that small detail, because small details usually make the whole picture.
