$10 Bandana Badges for Sale

Alright, due to the fact I can get these suckers done relatively fast (if I stay focused) and can do them anywhere, I decided I'm going to sell these bandana badges at $10 per badge.
There really is going to be an unlimited number of slots. As ambitious as it may be, I'm magically hoping to sell 50 badges by May 5th. Somehow. Though, honestly, I'll be estatic if I can even sell 10 of these. Anyways, if you guys want to help and try and spread word, go for it. If not, it's totally chill and understandable.
Below will be the order form. The process will be either note me on FA or e-mail me at hybridasylumstudios[at]gmail.com with the subject being "BADGE ORDER". Inside the body of the note/e-mail, please place a completed version of the following order form. When I get to it, I will read the note/e-mail and respond with whether or not I can take it. If I accept, I will ask that you send $10 to my paypal ASAP; the paypal account e-mail will be included in my reply.
EXTRA: If you wish to have a different fabric pattern or look to your bandana, do say so. A different pattern will cost an extra dollar, though. :v
